Student Computer Use Survey Form

In order to demonstrate to the state (a requirement of our digital high school grant) that Tamiscal students are using technology, please complete this form

• You need to enter your answers on a scantron sheet attached to your first week assignment packet.

Download the Computer Use Survey, and answer the questions.
• Use a #2 pencil.  No pens!
• You need to enter your answers on a scantron sheet. Answer questions 1-50 on the purple side, and 51-55  on the green side.  Answer the short answer question on the blank areas in the middle of the green side.There are extra scantron sheets in a folder on Mary's desk.
• If you need  to work with a printed copy, you may print a copy or ask Mary for one.
• Return the completed scantron sheet with your first week assignment.
